Pursuant to Article IX of the United Nine Workers of America 1950 Benefit Plan and Trust, and under the authority of an exemption granted by the United States Department of Labor, the trustees have received a question concerning the coverage of charges by your Employer’s Benefit Plan for the reversal of sterilization procedures.

Under Article III. A. (11) (a) 14 of your Employer’s Benefit Plan, coverage of charges for reversal of sterilization procedures is specifically excluded. Since this procedure is not a covered benefit under the plan, your Employer is not responsible for the provision of benefits for the reversal of your spouse’s sterilization procedure.
