Billing Instructions for Department of Labor (DOL) Black Lung benefits:
Claims for treatment of Black Lung for beneficiaries who have Black Lung benefits through the Department of Labor (DOL) must be billed to the DOL Black Lung Program first. A beneficiary who has coverage through DOL will have a Black Lung Benefits Identification Card (BLBIC).

For beneficiaries whose Black Lung benefits are paid by a Responsible Mine Operator payer (RMO), or programs other than the DOL Black Lung Program, bill claims to the payer listed on the ID card, if available, or contact the Funds at 800-291-1425 for assistance.
Services/Supplies Not Covered by Black Lung benefits:
For Funds’ beneficiaries that are eligible for Black Lung benefits, the Funds will process claims for services and supplies that that are not related to Black Lung disease. All claims must first be submitted to the Black Lung payer. If denied, a copy of the denial must be submitted to the pharmacy or medical claims office for Funds’ payment consideration.
