Geriatric Care Management (GCM) is a care coordination initiative designed to improve the quality of life of frail elderly beneficiaries and their families. The program seeks to enable beneficiaries to achieve maximal functional health status, increase access to primary and preventive care as a means to improve quality and control health expense trends, and change patterns of care in order to better meet the health and social needs of the beneficiaries and their families.
Geriatric Care Managers, nurses with extensive skills and experience in providing home health and related services to elderly and frail individuals, work directly with beneficiaries, their families, and their providers to facilitate access to health system resources and coordinate the delivery of care and other services. The program involves frequent on-site visits by the GCM nurses to the beneficiaries and their families
GCM services are currently provided to beneficiaries living in certain counties in West Virginia, Alabama and Utah.
GCM services are provided by various contractors. Program enrollment at any point in time is approximately 500 beneficiaries.
