On this page you will find information about health and pension benefits, how to apply for benefits, and Summary Plan Descriptions which will explain the various benefit types and rules for eligibility.

How to apply for Death, Health and Pension Benefits

You may get a death, health or pension benefit application by calling the Funds’ Call Center and from many UMWA district and local union offices. You can also find copies of these applications as well as other forms under Eligibility Forms on this site. You may also request the application from the Funds’ Call Center.

If it is difficult for you to fill out the required forms, a Funds’ employee will help you; there is no charge for this service. If you need such assistance, show the worker who is helping you all records that indicate when and where you have worked. You may also have your own representative assist you. When you fill out your application be sure to spell all names correctly and write your Social Security number and requested dates accurately. Include all material that is requested on the application such as copies of your birth certificate, military service records, and any disability or workers’ compensation awards that you have received. When listing work at a mine signatory to a coal wage agreement, note the local union number for that mine. -->

If you have not completed your application properly, the plan will have to contact you to get the needed information; this will delay the processing of your application. After you have completed the application, mail or take it to the Funds’ Washington, D.C. office.

The beneficiary call center can be reached from 9 – 4:45 Monday through Friday at 1-800-291-1425 to answer your questions about pension or health applications as well as any questions about your current health or pension benefits.

The Funds' field office in Beckley, W.Va. continues to provide:

  • Pension counseling services before an application has been filed
  • Appeals of denied benefits (other than disability pensions)

Application forms are available at all Funds' and District offices. In addition, these offices will continue to provide general information about pension benefits
