The Funds sponsor various types of support groups, physical activity programs and health education and health promotion activities. Many are offered on a regularly scheduled basis, such as monthly support groups for diabetes and Alzheimer’s disease. Other programs are offered at various times and locations throughout the year, such as the Chronic Disease Self Management workshops. The health programs are offered in the areas surrounding the locations of the Funds Field Health offices. The Funds also work collaboratively with local providers to promote community programs. The Funds have 12 offices located in eight states: They are:
