Health and Wellness Programs

The Funds is committed to assuring high quality, cost effective services to beneficiaries. The Health and Wellness programs it offers to beneficiaries are part of the Funds core value and belief that these programs can make a positive difference in the lives of beneficiaries.

The Funds sponsor a variety of programs to help beneficiaries maintain their health and independent functioning. Some of the programs, like Disease Management, provide up to date education on how to better manage chronic conditions; others, like the Field Health Programs promote healthy lifestyles and direct beneficiaries to supportive resources in their community; and yet others, like Case Management and Geriatric Care Management offer services to those beneficiaries who may be experiencing a serious acute or chronic illness or are frail and homebound. As part of its Health and Wellness initiatives the Funds also produce Health Education Brochures on various topics and send them to beneficiaries several times a year. All of the Health and Wellness programs are provided free of charge.
