The Field Health Programs are directed principally toward prevention, health education, health promotion, and related clinical programs and activities. Staff is located in Field Health Offices that are situated where they can best serve the needs of the Funds’ beneficiaries, and they are staffed by registered nurses, social workers, in-field beneficiary service representatives, and provider relations personnel.
The Funds’ health promotion, disease prevention, and health education activities are centered principally on diabetes, injury prevention, and adult immunizations. Each of the initiatives is consistent with the goals and objectives set out in Healthy People 2010, a blueprint to improve the nation’s health published by the HHS Office of Disease Prevention and Health Promotion. Consistent with pursuit of these goals and objectives, the Funds’ programs also promote physical activity, proper nutrition, and maintenance of a healthy weight.
Health promotion, disease prevention and health education activities aimed at beneficiaries include those listed below:
- Campaigns to promote pneumococcal pneumonia, tetanus, and annual influenza immunization;
- Ongoing health education seminars and communications to beneficiaries on subjects related to prevention and wellness;
- Sponsorship and participation in community-based health fairs;
- Establishment of disease-specific support groups such as the one for diabetes serving beneficiaries in Wyoming county, WV, the first of its type in the area;
- Notifying beneficiaries of local, community-based meetings/events, which address diabetes, fall prevention, physical activity and other health topics;
- Working with local businesses to support and contribute to home safety and home modification measures, such as smoke detectors and wheel chair ramps;
- Making home visits at the request of the Funds’ medical management contractor, medical providers, community agencies or family and friends to assess a beneficiary’s home environment and medical and social needs;
- Making home visits to assist caregivers to better understand the beneficiary’s medical conditions and to ensure they are aware of available community support resources;
- Identifying beneficiaries who have medical or social needs and making referrals to the Funds care coordination and disease management programs or to community agencies for services such as meals on wheels; and
- Assisting beneficiaries who are applying for assistance from federal programs (e.g., Medicaid).
