The objective of disease management is to improve beneficiaries’ quality of care and quality of life, and to manage on-going healthcare expenses by focusing educational, coordination and care monitoring activities on beneficiaries with specific or dominant conditions.
Currently, telephonic disease management programs are offered to Funds beneficiaries with congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), diabetes, and depression. These disease management programs provide educational materials and information related to the specific diseases and approaches to their management. The programs educate the beneficiaries about management of their diseases and monitor their progress and condition by telephone. The relevant physicians are advised of the beneficiaries’ participation in the program and the program’s content, and they are kept apprised of the beneficiaries’ status. The average length of stay for a beneficiary in these programs is six months.
Disease Management services are provided by Long Term Care Group nurses. Generally, there are almost 200 beneficiaries enrolled in one of the disease management programs on any given day.
